Group Agreement and Informed Consent

Welcome to A Year to Live. Before we begin our journey together, please read through the following agreements and disclosures carefully. This document ensures we have a shared understanding of the group's purpose and roles.

1. Professional Disclosure and Nature of the Group

Educational and Contemplative Focus This group is an educational and contemplative program centered on mortality awareness, mindfulness, and the existential themes explored in Stephen Levine’s literature. The purpose of this group is to foster community, personal reflection, and shared exploration of these topics.

Scope of Facilitation While the facilitators of this group are licensed mental health professionals, this is not group therapy or a clinical mental health treatment. The facilitators are operating strictly in the role of educators and guides, not as your personal or clinical therapists. By signing this agreement, you acknowledge that no therapist-client relationship is being formed within the context of this group.

If the material explored brings up significant psychological distress, resources may be provided to support members in receiving appropriate clinical care. Additionally, participants may schedule individual sessions with the facilitators for additional 1:1 support outside of the group format if needed.

2. About the Facilitators

  • Angie Frank, LLP, LPC (Inward, LLC): A licensed therapist with 15 years of experience, specializing in existential methods including mindfulness-based, nature-focused, and somatic therapies.

  • Stephanie Miller, LPC (Selah Mind): A mindfulness-based psychotherapist, integration coach, and meditation teacher with a private practice in Colorado, where she also leads retreats in the Rocky Mountains.

3. Program Logistics and Inclusions

This year-long group runs from January 8, 2027, through December 10, 2027. We will gather via Zoom on the second Friday of each month from 10:00 am – 11:30 am MT. The Zoom link will be provided via email before each session. 

Community and Materials

  • Care Package: Prior to our start, and if you opt to share your address, you will be mailed a package including Stephen Levine’s A Year to Live, the Five Wishes advanced directive, a printed version of the Five Remembrances, and a journal. 

  • Community Space: You will have access to a dedicated community space to share reflections, stay connected, and support one another in the ongoing practice of genuine presence between our monthly meetings. Please note that the facilitators are minimally involved in this community space as it is primarily a space for participants to connect. 

4. Attendance and Participation

We recognize that life is unpredictable and that committing to a year-long process requires flexibility.

  • Attendance: There is no penalty or pressure for missing a gathering. If you cannot make a Zoom session, you do not need to notify us in advance. We will simply hold the space until you are ready and able to return.

  • Voluntary Withdrawal: If at any point you decide this group is no longer serving you, you are free to step away from the year-long journey without judgment (subject to the financial agreements below).

5. Group Agreements

To create a supportive, respectful, and deeply connected environment, we ask all participants to commit to the following agreements:

  • Confidentiality: You are welcome to share your own insights outside of the group, but agree to never disclose the identities, stories, or personal details of other participants.

  • Listen Deeply & Speak from the "I": We are here to hold space and share from the heart using "I" statements, without offering unsolicited advice, interrupting, or trying to "fix" one another.

  • Share the Space: We gently invite more talkative individuals to practice restraint and quieter individuals to practice stepping forward, ensuring there is room for everyone.

  • Pacing & Presence: You are the expert on your own nervous system, so please minimize distractions and feel completely free to step away, turn off your camera, or opt out of sharing whenever you need to.

  • Compassion & Repair: Assume everyone is doing their best, embrace perspectives different from your own, and remain open to connection and repair when unintended impact occurs.

6. Financial Agreements

  • Investment: Tuition is offered on a sliding scale from $800 – $1,200.

  • Deposit and Registration: A $200 deposit is required to reserve your spot. Payment may be made on the registration site: www.seeinward.com/register or via Venmo or check mailed to Angela Frank at PO Box 111, Lyons, CO 80540

  • Payment Schedule: Full payment is due by December 17, 2026. If you would like to discuss a payment plan, please reach out to us directly.

  • Refund Policy: A full refund is available until January 10, 2027. A 50% refund is available until January 31, 2027. After January 31, 2027, the course is strictly non-refundable.

7. Assumption of Risk and Release of Liability

By participating in this group, you acknowledge that engaging in deep emotional and existential reflection carries inherent personal risks, including the potential for emotional distress. You voluntarily assume these risks.

You hereby release, waive, and discharge the facilitators and any affiliated staff from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, or emotional distress that may be sustained by you as a result of participating in this group.

By clicking “I consent” upon registration I agree that I have read and fully understand this Group Agreement and Informed Consent. I understand that this is an educational group, not psychotherapy, and I agree to the financial, attendance, and behavioral policies outlined above.
